The 5-alpha reductase enzyme family is responsible for the conversion of testosterone to DHT. With cofactor nicotinamide adenine dinucleotide phosphate (NADPH), isoenzymes of the 5-alpha reductase enzyme family catalyze an irreversible break of the double bond between carbons 4 and 5 of the testosterone molecule resulting in conversion to the DHT molecule.
